PrimeCare Medical is currently hiring a Community Counselor to provide counseling and support services for individuals living with HIV, those at risk, and affected family members throughout the Hampton Roads area.

This role focuses on improving mental health, reducing stigma, and helping individuals access healthcare services and community resources. The counselor will work directly with clients to provide education, support, and linkage to care.

Responsibilities

• Provide one-on-one counseling sessions for individuals living with HIV, those at risk, and affected family members 
• Address mental health concerns, stigma, and coping strategies related to HIV 
• Provide counseling services to approximately 40 patients per week 
• Conduct HIV risk assessments and screenings 
• Educate individuals on safer sex practices, testing, and prevention methods 
• Assist clients in navigating HIV-related healthcare services including linkage to care and treatment options 
• Support medication adherence and treatment engagement 
• Provide information about support groups and community resources 
• ,Promote empowerment, resilience, and emotional wellness among clients

Requirements

• Background in counseling, social work, public health, or a related field preferred 
• Experience working with HIV/AIDS programs or community health services is a plus 
• Strong communication and interpersonal skills 
• Ability to provide compassionate, confidential client care 
• Ability to manage multiple clients and maintain documentation

Schedule

Monday – Friday 8:30 AM – 5:00 PM
